    Medical

     

     

    Local Medical assistance may

    Include:

                                         

    1.      One- Office call

     

    2.      Prescriptions: $500.00 Maximum per

                               six month eligibility period

     

    3.      $500.00 toward in-hospital physician

     

    4.   Hospital Admission: $500.00 Maximum

     

                                                    All doctor appointments must be

    scheduled by the consumer and

    reported to the General Relief Office.

     

           All interested consumers

                                                 Must apply every month!

                                                             

                                

                                                 Appointments are scheduled

                                                 with Elma or Tammy.
