                       REFERRAL PROCESS

     

                              

     
         Access points should refer to Case Management             

                        Services or CPC office for access to any services

                    outside of level one or two services, such as RCF

                        or vocational services.  Level one services referrals

                        may be directed to designated outpatient service

                        provider and level two services directed to Keokuk

                        Area Hospital or Mental Health Institute (using pre

                        screen as needed). Case Management services may

                        directly refer for other services, or may refer to CPC

                        Administrator for additional assessment and ICP

                        planning. 

               

                        Case Management services receives referrals from all                   

                        sources, and may provide intake and information and        

                        referral services for all applicants.  For those eligible         

                        for services, case management determines appropriate      

                        payment source.  If the referral is MHAP, referral is         

                        made to MHAP for further assessment and ICP planning.

                        If the referral appears to need level four or five services,

                        case management performs an intake, then contacts the 

                        CPC administrator before proceeding.  If Central Point of            

                        Coordination Administrator approves, county case

                        management will proceed to complete assessments and

                        treatment planning, and will initiate referrals to

                        appropriate providers.  Once a ICP is in place, county

                        case management will present the plan to the Central

                        Point of Coordination Administrator for final approval.
