consumer PLAN DEVELOPMENT
Once an individual is enrolled as an eligible consumer,
ICP Planning
will commence. As noted previously,
if
there is
a waiting list for the desired service(s) due to
funding,
all communication and planning will be conducted
through
the CPC. Also, any ICP that includes some
form
of level
four or five service must be approved by the CPC
Administrator. For these cases,
the CPC Administrator
may elect to assign county case management to perform
the necessary assessments and ICP Planning. However,
in these
cases the CPC Administrator must still give final
approval to the ICP.
ICP Planning and authorization at Case
Management
If an individual presents for service at county case
manage-
ment (or is referred to case management by
another access
point), meets threshold criteria,
and is in need of case
management as well as other services, case management may
prepare an ICP and authorize services, with the exceptions
noted above. For level one and
three services, county case
management may
process all service authorizations, and then
notify the CPC Administrator of enrollment and service
authorization.

authorization aT
OUTPATIENT SERVICE PROVIDERS
Individuals presenting at the
outpatient service providers may
be
processed for outpatient services by their clinical staff. If
other out-patient services (such as day treatment), are
indicated, their clinical staff will prepare a treatment plan and
present it to case manager or CPC Administrator. In many
of these cases, a need for case management will be indicated,
and thus the CPC
administrator will refer to case management
for
further assessment and ICP planning.
If only outpatient
services
are needed, and if case management is not indicated,
then the
CPC Administrator may authorize the treatment plan
for the outpatient service provider.
ICP PLANNING AND AUTHORIZATIONS AT OTHER
LOCATIONS
For individuals presenting at either the hospital or the courts,
the intake will be completed
and forwarded to the CPC.
Emergency
services may be initiated, however, no other long
term planning or authorization
may take place at these
access points.
In many cases, consumers will be referred by the
CPC or
county case management to one or more service providers.
In these circumstances, the receiving provider may be invited
to participate in ICP Planning.
Informed
Consent
In all cases, the primary consumer will be asked to give
informed consent and approval to the ICP. If the consumer
is an adult, and has no guardian or conservator, s/he may
elect to involve family
members the ICP Planning process,
and to approve the final
ICP. If the individual is a minor,
has
a guardian or
conservator, or is otherwise unable to give
informed consent, the designated guardian, parent, or other
representative will approve the ICP.
In all cases, consumers
may be represented by advocates, other consumer
representatives,
friends or family during the ICP planning
process.
County of legal settlement and state
cases
If the consumer has a county of
legal settlement that elects to
perform the CPC functions for
out-of-county consumers,
then the CPC Administrator will
coordinate ICP Planning
with the county of
settlement. However, it will be the
policy
of Lee County to assure that individuals requesting services
from the county of residence
(in this case, Lee County), will
receive the same degree of access to services contained in the
county management plan as will
individuals for whom Lee is
the
county of legal settlement. However,
services authorized
to persons with state case status will be subject to the funding
limitations established by the
state.
