                     REQUEST FOR FUNDING

        

     

             

               If a service is requested not currently cover

               in the county management plan, or services                  

               from an out-of-area provider, the request must

               be presented to the County Board of Supervisors

               for approval.  The request will be developed by

               the CPC Administrator, ordinarily with the assistance

               of a case manager.  The plan must then be presented

               to the county Board of Supervisors for review and

               approval.  For all such requests, Board of Supervisors

               re-authorization will be necessary at least every six months. 

     

     

               If the consumer has a county of legal settlement that

               elects to perform the CPC functions for out-of-county

               consumers, then the CPC Administrator will coordinate

               planning with the county of settlement

     

                 

               For individuals with state case status, the county will work

               with the local DHS office and/or MERIT to develop and

               approve the ICP.

     

               

               As noted in other sections of this plan, county of legal

               settlement or state case status will not be a factor in

               initiating crisis stabilization/hospital diversion services. 

               Further, Lee County will make every effort to initiate

               appropriate services while determining the county of

               legal settlement and receiving the cooperation of the

               CPC in the county of legal settlement. Lee County does

               not intend to deny access to necessary services for any

               current resident of the county, and will work to assure

               that counties of legal settlement and DHS cooperate in

               funding appropriate access to services.
