    SERVICE MONITORING

     

                                                                             

     

                           The Management Information System (COMIS) developed

                           by the Department of Human services is used for data

                           collection and planning.

                                                                                                   

                                                              

                           As noted elsewhere, the central consumer enrollment file will

                           be maintained by the Central Point of Coordination (C.P.C.).

                           All intake and enrollment data will be forwarded to the Central

                           Point of Coordination for data entry.

     

                           Upon intake, consumers or their representatives will be asked

                           to give informed consent that intake information be provided by

                           the access points to the Central Point of Coordination office. 

                           This is essential in order to assure that currently enrolled

                           consumers will be identified if they present at any of the access

                           points in the county managed system of care.  However,

                           consumers will be assured of strong confidentiality protection,

                           and no clinical records will be available through the enrollment
