    Plan Development Process

     

                     Description of the process for obtaining consumer,

                     family, and other stakeholder input in the planning

                     process:

     

            Process for selection of a Stakeholder Planning Committee:

     

                     When the plan was originally developed in 1996, three open meetings

                     were held to solicit input and identify persons interested in volunteering

                     for service on the stakeholder group.  The first open meeting was for

                     consumers and their families, and was held on January 25, 1996. 

                     Consumers were notified through newspaper and radio announcements,

                     as well as a special notification through Case Managers.  Food and

                     limited transportation was provided, approximately 30 individuals were

                     in attendance.

     

                     A second meeting was held for providers on February 6, 1996, with

                     seventeen individuals representing six organizations in attendance.

     

                     A third meeting was held on February 23, 1996 for representatives of

                     organizations such as schools, Public Health, Voc Rehabilitation and

                     Department of Human Services who are advocates for consumers but

                     who are not County funded providers. Four persons were in attendance.

     

                     From these three meetings, volunteers agreed to serve on

                     a stakeholder committee to write the original plan.  Due to the

                     success and dedication shown by this group, the Board of

                     Supervisors decided to maintain the stakeholder group on an

                     ongoing basis to advise the Board.

                     As the result of a group consensus building session,

                     the following mission and belief statements were adopted:

     

           LEE COUNTY MISSION STATEMENT FOR PERSONS WITH

           MENTAL ILLNESS, MENTAL RETARDATION, AND DEVELOPMENTAL

           DISABILITIES

     

                    IT IS THE MISSION OF LEE COUNTY THAT PERSONS

                    WITH MENTAL ILLNESS, MENTAL RETARDATION, AND

                    DEVELOPMENTAL DISABILITIES BE EMPOWERED TO

                    ACCESS THEIR CHOICE OF APPROPRIATE, INDIVIDUALIZED

                    SERVICES DELIVERED IN THE LEAST RESTRICTIVE SETTING

                    WITHIN THEIR COMMUNITY.

     

                    LEE COUNTY WILL PROVIDE A COMPREHENSIVE SERVICE

                    DELIVERY NETWORK WHICH EMPHASIZES QUALITY,

                    FLEXIBILITY, AND COST EFFECTIVENESS THROUGH 

                    INTERAGENCY AND REGIONAL COOPERATION.

                 

                   On behalf of the citizens of Lee County with Mental Illness,

                    Mental Retardation and/or Developmental Disabilities, we believe:

     

           WE BELIEVE IN THE SYSTEM PRINCIPLES OF:    

        

                    “CHOICE” WHICH MEANS THE ABILITIES OF

                     CONSUMERS, THEIR FAMILIES, AND AUTHORIZED

                     REPRESENTATIVES TO EXERCISE INFORMED

                     CHOICES ABOUT THE AMOUNTS AND TYPES OF   

                     SERVICES AND SUPPORTS RECEIVED.

     

                     “COMMUNITY” WHICH MEANS THAT THE SYSTEM

                     SUPPORTS THE RIGHTS AND ABILITIES OF ALL

                     CONSUMERS TO LIVE, LEARN, WORK, AND RECREATE

                     IN NATURAL COMMUNITIES OF THEIR CHOICE. 

     

                     “EMPOWERMENT” WHICH MEANS THAT THE

    SERVICE SYSTEM REINFORCES THE RIGHTS,

    DIGNITY, AND ABILITY OF THE CONSUMERS AND

    THEIR FAMILIES TO EXERCISE CHOICES, TAKE

    RISKS, PROVIDE VALUABLE INPUT, AND ACCECT

    RESPONSIBLITIES.                                                                      

     

     WE BELIEVE IN DEMONSTRATING COMPASSION, RESPECT,

     AND DIGNITY FOR ALL INDIVIDUALS.

     

           WE BELIEVE IN COMMUNITY RESPONSIBILITY IN PROVIDING

     SERVICES FOR THESE INDIVIDUALS.

     

           WE BELIEVE IN MAINTAINING EXISTING QUALITY

     COMMUNITY BASED SERVICES.

     

           WE BELIEVE IN AND SUPPORT SERVICES IN THE

            LEAST RESTRICTIVE ENVIRONMENT.

     

     WE BELIEVE IN THE IMPORTANCE OF FAMILY SUPPORT.

     

     WE BELIEVE IN THE ACCESSIBILITY OF DECENT AND

     AFFORDABLE HOUSING FOR THESE INDIVIDUALS.

     

     WE BELIEVE THAT ADEQUATE TRANSPORTATION IS

     ESSENTIAL TO THE SUCCESS OF INDIVIDUALS LIVING

     IN THE COMMUNITY.

     

     WE BELIEVE IN THE IMPORTANCE OF PROVIDING A

     FINANCIAL ENVIRONMENT THAT WILL ALLOW PROVIDERS

     TO ATTRACT AND MAINTAIN AN ADEQUATE NUMBER OF

     QUALIFIED SERVICE STAFF.

     

     WE BELIEVE IN QUALITY, COST EFFECTIVE SERVICES
