    Brochures with information on each vaccine are provided prior to immunization of each individual. Please read and ask questions prior to seeing the nurse.

    If a parent is unable to attend, a signed permission slip must accompany each child. Please bring your child's immunization record to the clinic. We cannot give your child immunizations without it. A complete immunization record is required by State Law, which includes the date, the shots given and the physician's or clinic's name.

    It costs our Agency $5.00 to immunize each individual. Please help us to cover the costs with a donation if able. Thank you.

    FREE lead screening is offered at all immunization clinics or by appointment. Children between the ages of 6 months and 6 years are at risk of lead poisoning and should be screened.

    Untreated lead poisoning can result in:

    • Learning Disabilities
    • Poor Attention Span
    • Behavioral Problems
    • Mental Retardation
    • Permanent Brain Damage
