Requesting a Certified Copy of an Iowa Vital Record

Iowa Vital Records are official registrations of births, deaths and marriages. Certified copies of Vital Records can be obtained from a County Recorder’s office or the Iowa Department of Public Health - see chart below. Statewide record searches are available from the Iowa Department of Public Health.

County Recorders maintain births and deaths that have occurred in that county and file and maintain marriages of the licenses that were applied for in that county. However, with the implementation of the statewide system, county Recorders have access to certain records in all counties (see chart below). The fee at the county level is $15.00 for each certified copy. Entitlement has to be proven before a certified copy can be issued.

If you mail in our application, you will need a money order made payable to "Lee County Recorder" unless you are sending our form to Des Moines. In that case, you would need to make your check or money order payable to "Iowa Department of Health". If you have any question regarding payment, please call the Recorder's office at 319-376-1070.
